RELEASE OF LIABILITY & INFORMED CONSENT
Exercise Participation & Nutrition Coaching
I voluntarily choose to participate in a fitness training program and/or nutrition coaching provided by Eric D. Haun / Eric Haun Fitness (“Coach”) to improve my health and physical fitness.
Exercise Participation
I understand that participation in exercise involves inherent risks, including but not limited to abnormal cardiovascular, respiratory, or musculoskeletal responses. I acknowledge that these risks cannot be completely eliminated.
I agree that I am solely responsible for monitoring my physical condition during all workouts and assessments. I will immediately stop exercising and notify the Coach if I experience pain, dizziness, shortness of breath, chest discomfort, or any unusual symptoms. I understand that I may discontinue participation at any time.
Medical clearance from a physician is recommended prior to beginning any exercise program. If medical clearance is required by the Coach, I agree to obtain it before participating.
Nutrition Coaching Disclaimer
I understand that nutrition coaching is provided for educational and informational purposes only and is not intended to diagnose, treat, cure, or prevent any medical condition or disease.
I acknowledge that the Coach is not a licensed medical provider or registered dietitian, unless otherwise stated. Any nutritional guidance provided is based on general principles of healthy eating and is not a substitute for medical advice.
I agree to consult with a qualified healthcare professional before making significant dietary changes, especially if I have a medical condition, food allergy, or am pregnant or nursing.
I accept full responsibility for my dietary choices, reactions, and results.
Assumption of Risk & Release
By signing below, I confirm that:
I have read and fully understand this Release of Liability and Informed Consent.
All questions have been answered to my satisfaction.
I voluntarily assume all risks associated with exercise and nutrition coaching and release and hold harmless Eric D. Haun, Eric Haun Fitness, and all affiliated parties from any and all claims, demands, injuries, losses, or damages arising from my participation, except in cases of gross negligence or willful misconduct.
